You also need to put "headdress, helmet, headwear" in the negative prompts for it to come out completely. At 1 strength it'll stay on there 95% of the time. Finally if you can't remove the gloves or neckwear, then put "bare arms, bare neck, bare hands" in the positive prompts and that should help.
