First-person point of view looking steeply down at a glass desk counter that fills the lower two-thirds of the frame, the camera tilted so the counter's far edge runs diagonally across the upper third. In the immediate, razor-sharp foreground, the viewer's own forearm lies across the glass from the lower left, wrist turned palm-up; two long, elegant fingers -- index and middle, of a pale surgeon's hand -- descend at an angle across that bare wrist and trail slowly downward toward the pulse point, fingertips pressed flat with visible slight blanching of the skin under the pads, the wrist cords and the faint blue vein lifting under the touch, one fingertip paused mid-slide. The hand is bone and tendon and clean short nails, kept unadorned, no ring, no watch, no bracelet, nothing else at the wrist; the touch is deliberate and lingering, fingertips angled so the side of the index traces the tendon line. Resting on the glass beside the wrist, softly separated from the sharp zone: a light blue surgical mask pulled off and dropped, one ear loop flipped over on itself in a crumpled oval, its inner pleats visible, the fabric still holding the crease of a worn face; next to it a thin silver penlight lies on its side, its metal barrel catching a hard white highlight, the button end toward the viewer, the lens tip dark. At the back of the frame the surgeon is reduced to a blurred mass -- an open half-unbuttoned white shirt over a bare chest, collar folded back, the button placket hanging open in two soft vertical bands, warm skin in the V between them, no tie, nothing at the throat; the blurred chest is large behind the fingers and out of focus, only shape and tone. The viewer's wrist and the surgeon's fingers are the only sharp things; the mask and penlight sit a step softer, the chest and the far end of the counter dissolve. Lighting is a single cold overhead source from above and slightly behind the surgeon, so the glass throws a soft doubled reflection of the penlight and a dimmer smear of the mask downward toward the front edge, and the surgeon's body lifts a long soft shadow forward across the counter; the wrist holding the pool of light, the hollows of the anatomy deepening into warm shadow. Palette restrained to surgical cool -- glass grey-green, clinical white, pale blue of the mask and one faint warm tone in the blurred skin -- silky glass highlights, matte fabric, glossy short nails, high-fidelity 3D rendering with visible subsurface scatter on skin and a shallow depth of field that thins into real bokeh at the chest.
